Process safety (PSM)

Management of change, incident investigations, actions, process information, procedures and chemicals, under the standard you choose.

At platform.beedy.ca/psm/. The tabs: Overview, Change (MOC), Incidents, Actions, Process information, Procedures.

One program, several standards

Choose the Standard lens: CSA Z767:24 (the default), OSHA 29 CFR 1910.119, CCPS Risk Based Process Safety, Seveso III (2012/18/EU) or EPA RMP 40 CFR 68 (Program 3). The program stays the same when you switch; the lens changes the element names and references. The demonstration plant's chemicals reach none of the regulatory thresholds, so it is run as good practice.

Management of change (MOC)

A change moves through: Draft, Screening, Risk review, Approval, Implementation, Pre-startup review (when required), Close-out.

StepWho can move it
DraftAnyone
ScreeningProcess engineer
Risk reviewProcess engineer or HSE coordinator
ApprovalBoth the HSE coordinator and the plant manager
ImplementationMaintenance lead, process engineer or operator
Pre-startup reviewHSE coordinator or process engineer
Close-outProcess engineer or plant manager

A change can be rejected during screening, risk review or approval. It cannot close while its actions are open. Temporary changes carry an expiry date, and OTIS flags one that has expired while still open. Types: equipment, process, chemical, procedure, operating limit, organisational, control or software.

Incident investigations

Closed incidents on the live plant without an investigation are listed. Draft investigation creates one with the what-moved-first timeline and the crew's notes already in. Steps: Draft, Triage (the 48-hour clock starts here), Investigation, Actions, Review, Closed. A classification is required before investigating, a method and root causes before actions (5 whys, causal tree, fishbone or barrier analysis), at least one corrective action, and only the plant manager closes the review.

Actions

Open, In progress, Awaiting verification, Closed (or Rejected with a written justification). Someone other than the owner verifies and closes. Extensions are at most 90 days each, with a reason.

Process information and operating limits

Mark reviewed sets the next review two years out. To change a safe operating limit, click Propose change: it opens an operating-limit MOC, and the new limit applies only when that MOC closes.

Procedures

Procedures are certified annually, by the plant manager or a process engineer. Emergency procedures are flagged.

Indicators

Tier 1 and 2 events are classified by people during investigation. Tier 3 is computed from the historian over the last seven plant days (limit excursions). Tier 4 tracks the management system: actions on time, procedures certified, process information current, investigations started within 48 hours, expired temporary changes.

Roles

Use Acting as to switch between plant manager, process engineer, operator, maintenance lead and HSE coordinator. Every button is visible to every role; the server refuses a step that is not yours, and the page shows why.
