PSM · Module 9 of 12

Investigate to learn, not to punish

Every incident and every near miss is information the plant gives you about its barriers. A good investigation turns it into a lasting fix; a bad one finds a culprit, closes the file — and waits for the repeat. As a manager, you decide which of the two your plant does.

≈ 70 minWorkshop: timeline + 5 whysInteractive fishboneQuiz: 8 questions
By the end of this module you will be able to:
  • say which events must be investigated, how fast and with which team (OSHA 1910.119 (m) and BTI-PSM-13);
  • classify a near miss by its potential, not by its luck;
  • tell immediate cause, causal factor and root cause apart, and not stop at "human error";
  • pick the method (5 whys, timeline and causal factors, fishbone, barrier analysis) according to severity;
  • build a just culture where people report, and close the loop: recommendations, effectiveness check, sharing lessons.
